Social prescribing is a way of linking you to non-clinical support in the community. Improving your lifestyle and making changes can be very challenging. You may not be sure where to start. Social prescribing can help to give you the confidence and support you might need to make a change.
There are a fantastic range of activities and organisations in the local community. Social prescribing is about linking you to these opportunities, with the aim of improving your health and wellbeing.

Emer Clark
As a Social Prescriber I work with children and young people to find out what matters most to them. I am here to listen and support young people in a way that empowers them and provides a space for them to be themselves..
I have previously worked in various settings, including a forest school, intergenerational arts charity, youth group charities and have an occupational therapy background. I love being outdoors and am happiest when sat by a campfire or playing in the sea..

Jess Careswell
I am interested in listening to patients and finding out what matters to them and how I can help them to reach their goals.
It is energising to be part of a team which has knowledge of services available in the area that can link people to community resources. In my spare time I like to go walking and spend time with my friends and family.

Katie Hayne
In my role as Social Prescriber, I enjoy connecting with people from all walks of life, with a focus on building meaningful relationships.
Using my previous skills as a support worker I want to help people achieve their goals and guide them to the right services that support their needs. I have travelled extensively and enjoy diverse perspectives and challenges.
